Travel Requirements

Nursing Requirements
Required Documents -
• Current nursing license(s)
• Annual physician's statement including PPD (CXR required for positive result),
Hepatitis B immunity, Varicella Zoster, MMR or Rubella, Rubeola, and Mumps Titer
• Current BCLS certification
• Proof of citizenship or eligibility to work in the United States
• ACLS where required
• PALS and NRP where required.
Allied Health Requirements
Required Documents -
• Proof of citizenship, or eligibility to work in the United States
• Health records (MMR, TB, Hepatitis B, and recent physical)
• Recent CE course certificates
• Current certifications and state licenses
• Resume, work history, and references
• Completed application, current references, resume and work history
